Transition

 
Leaving school and moving into adulthood can be a challenging time for young people and their parents. What kinds of choices exist and will they help the young person lead a fulfilling life?

Many parents feel anxious at this time, predicting poor services or a possible lack of choice for their children.

The first Annual Review following your son/daughter’s 14th Birthday is called the 14+ Transition Review, whereby all involved start looking at a Transition Plan. A pupil centered plan is discussed and developed over the next few years.

The plan is to identify your child’s education and support needs after 14 years of age.   Staff at Pen-y-Cwm help find out and record what is important to the young person now, and in the future.

Some things are easier for young people to think about:

  • What do I like doing to have fun?
  • Which lessons do I like in school?

Some things are much harder for young people to think about:

  • Where do I want to live when I am an adult?
  • What sort of job will I be able to do?
  • How will I get about?
  • What money will I need?

The Transition Plan we use in Pen-y-Cwm school is broken down into sections so that you and your son or daughter can think about each section when you are ready.
